The hidden $12B bottleneck in U.S. medical imaging:

Unless you only see one single healthcare provider in your lifetime, getting your prior medical images at different provider sites is an extremely complex and laborious process. The current requisition process typically involves:

  • Manual Identification: Providers must first determine where a patient’s prior imaging studies were conducted. This often requires contacting multiple facilities to locate the necessary images.

  • Request Submission: Once the imaging facility is identified, providers must submit a formal request, which can be time-consuming and requires detailed patient information.

  • Waiting for Response: There is often a delay as providers wait for the imaging facility to respond, which can take several days.

  • Data Transfer: The transfer of imaging data is typically done through physical media such as CDs, which adds an additional layer of complexity and potential for error.

Despite $100 billion annually spent in the US across 700 million medical imaging procedures, the lack of a national standard for medical imaging exchange in the U.S. results in significant inefficiencies and costs, with an estimated $12 billion a year spent annually to aggregate imaging data for specialist stakeholders in care management.

Ali Bakhshinejad, PhD, CEO of NexusMD, unfortunately had to experience this firsthand:

In mid-2021, Ali’s wife received the life-altering news that she had thyroid cancer. What should have been a straightforward process of seeking a second opinion quickly turned into a frustrating ordeal. They faced months of chasing down medical records and prior imaging, only to receive incomplete data—300 pages of PDFs without the critical images needed for a thorough evaluation. The result? A 4 month delay in treatment, repeated tests, and an overwhelming $15,000 in medical bills, all of which could have been avoided had they had access to their initial medical images.

This deeply personal experience underscored the glaring inefficiencies in the medical imaging requisition process and inspired the creation of Nexus MD. The company was born from a simple yet profound realization: "We can do so much better." Nexus MD aims to ensure that no patient or family has to endure the same unnecessary hardships, delays, and expenses that they did during their journey.

Don’t just take our word for it though. A diagnostic radiologist colleague and frequent coinvestor of ours had this to share about NexusMD:

NexusMD is solving a burning problem for radiologists/orthopedic surgeons, other docs, patients, and support personnel/healthcare systems. As a practicing radiologist, I am hampered daily by a lack of access to prior imaging. It leads to decreased quality of reads, forces patients and support personnel to spend time and effort finding old images, and costs money and inconvenience when many patients are compelled to be reimaged. A "Microsoft Windows" level of medical imaging inter-accessibility would be a game changer for the system and Nexus-MD is poised to be the solution.

We believe NexusMD fits the need our colleague sees in his practice perfectly.

Why We’re Excited About Nexus MD:

Entering a massive market with a unique wedge: The U.S. imaging market exceeds $100B annually, with >700M studies each year across 41,000 sites. This company began with the hardest part: automating image requisition. That wedge gives them agency over data acquisition instead of relying on third-party networks. With this foundation, they’ve expanded into a full AI-powered clinical automation platform with modules for requisition, prior auth, QA, predictive analytics, and a patient-facing app.

Defensible AI positioning: Today’s AI imaging startups depend on access to data controlled by incumbents or third parties. By solving requisition first, this company controls the flow of imaging data end-to-end — giving them a defensible data moat that makes their AI modules more valuable and sustainable than competitors chasing hype.

Unique Dual-Sided Flywheel. NexusMD combines a B2B wedge in provider workflows with a B2C channel through the MyImage app. Clinics adopt the platform because it automates prior image requisition, saving staff time and reducing duplicate scans. Patients adopt the app because it gives them direct access to and control over their imaging history. Each side strengthens the other: patient-driven uploads accelerate provider adoption, and provider integrations funnel more patients onto the app. This dual flywheel accelerates growth and creates a defensible network effect across the imaging ecosystem.
